Refills may be requested Monday through Thursday from 8:00am to 4:00pm and Friday from 8:00am to 1:00pm.
Please call in your request at least forty-eight (48) hours before your prescription runs out. Refill requests should be left on the provider’s voice mail.*
The following information is needed:
- Date of birth
- Phone number where you can easily be reached
- Name of medication
- Name and location of pharmacy
We will only contact you if there is a problem. Please contact your pharmacy to see when your medication is ready for pick up.
* Some narcotic and other
controlled-substance prescriptions cannot be filled over the
telephone. You may have to come into the office and pick-up
a paper prescription.
