Referrals

Due to the complexity of the referral process within each insurance company, please allow 7  business days to complete each referral.

Chelmsford Primary Care (CPC) is affiliated with Lowell General Hospital. All referrals and specialist appointments will be confirmed with doctors within our network. All visits must be pre-approved by your PCP.  Any visits with specialists not within our network may not be allowed, and the cost of that appointment can and may be your own financial responsibility.

Not sure if your specialist is allowed? Call the office BEFORE you book anything.
 

Call Chelmsford Primary Care (978-256-5522 x30) to request the referral. You will need to provide:
• Name of your specialist and the group name if applicable
• Date of your appointment
• Reason for your visit
• Amount of visits needed
 

When you leave a voicemail message, clearly leave this information along with your name (correct spelling), date and birth, and a valid telephone number. You may also request a referral via the Patient Portal.
 

Last minute requests may affect your financial responsibility for that appointment.
